CADC-I Application Checklist
To obtain CADC-I certification, the following items are required:
- Completed Application & Portfolio Forms – Must include applicant and supervisor signatures.
- Resume of Prior Work Experience – Documented work history in the field.
- Official Transcripts – Must be mailed directly from the college/university to MAAP, along with a copy of your diploma and any additional certifications.
- Completed Education Resume – Must include adequate documentation supporting a minimum of 300 education hours, including 6 hours in Ethics (specific for Addiction Professionals).
- Written Case – A completed case study demonstrating knowledge and application of counseling skills.
- Ethical Standards Affirmation – The entire document must be dated and signed.
- Application Processing Fee – Must be submitted with the application.
- Current MAAP Membership – Membership must be active at the time of application submission.
- SPT Verification, Supervisor’s Evaluation, and Personal Reference Forms—These forms must be provided to the appropriate individuals and emailed separately to Certification@MSAAP.net by the supervisor and referees.
- Supervisor’s Letter of Response – If applicable, a letter from the Certification Board granting authority to supervise.
- Supervised Practical Training (SPT) Requirements:
- 200 hours for CADC-I applicants with a Bachelor’s Degree

Document Submission
All required documentation should be uploaded to your Member Portal or emailed to certification@msaap.net.
Exam Eligibility & Fee Payment
- Once an applicant’s application packet and portfolio are approved and the exam fee has been paid, they will receive notification of their eligibility to take the exam specific to their credential.
- The exam fee may be paid when submitting Membership and Application Processing Fees or after the Certification Board approves the application.
