CPS Application Checklist

To apply for the CPS (Certified Prevention Specialist) credential, the following items are required:

• Completed Application & Portfolio Forms – Must include applicant and supervisor signatures.
• Resume of Prior Work Experience – Documented work history in the field.
• Official Transcripts – Must be emailed directly from the college/university to MAAP, along with a copy of your diploma and any additional certifications.
• Completed Education Resume – Must include adequate documentation supporting a minimum of 150 education hours, with the following required coursework:
  • 75 hours in Prevention
  • 75 hours in Alcohol & Drug Use/Misuse
  • 6 hours in Ethics
  • 6 hours in HIV/AIDS training
  • 4 hours in Disruptive Audience training
• SPT Verification, Supervisor’s Evaluation & Personal Reference Forms – These forms must be provided to the appropriate individuals for separate emailing to MAAP at Certification@MSAAP.net.
• Ethical Standards Affirmation – The entire document must be dated and signed.
• Application Processing Fee 
• Membership Dues (if applicable).
